The following is the requirement for attestation of PAN Card application. Please note that the Consulate attests documents issued by the Government of India or State Governments in India and US State Authorities only.Document issued by the private companies and individuals will not be attested by this Consulate.

This is important

  • Documents issued by local authorities (USA) must initially be got authenticated (apostil led) by the Dept. of State of the State (where applicant resides) before applying for attestation in the Consulate.
  • Documents issued/originating from India, need authentication by the Ministry of External Affairs in New Delhi before presenting to the Consulate.

 

1.       Application form
Misc. Services form, duly filled in and signed (no column left blank) should be submitted. If the name of the service is not covered by the given options, indicate against “Other” name of service required. Please click below for downloading Misc. Services form.

2.       Photographs
One recent colour photograph of 2” x 2” against white background showing full frontal view of full face is required. This should be pasted (do not staple) on the first page of the application in the space provided for the purpose.

3.       Photocopy of Passport: Copies of first three and last two pages of the passport and any other page containing Passport Office’s observation. If you are submitting application by mail then send duly notarized copies.

4.       Proof of residence:
Any two of these i.e. copy of utility bill (phone bill /electricity bill) or Driving License or Lease Deed (of residence) etc. where your USA address is clearly mentioned should be provided. If you are sending application by mail then copies should be duly notarized.

5.       Visa Status
Copy of Permanent Resident Card (both sides) or Employment authorization card or copy of page showing H1 / H4, L1/L2, F1/F2 & J1/J2 etc. visa should be enclosed. If you are sending application by mail then copies should be duly notarized. Visa status is not applicable to US citizens.

6.       Document to be attested: Please provide the original and a copy of each document to be attested. This normally includes Passport, state ID cards including driving license etc, PAN card application form. Please note that PAN Card application should not be apostil led.  

6A. Copy of latest cancelled Indian Passport or a Copy of Surrender Certificate should be brought together with application.

7.       Fees:

(i) Fee: US$ 10/- for each attestation for normal applicants and US$ 50 for Commercial/Company documents.

(ii) Mailing fees: US$ 20/-
Surrender certificate fee: Please note that if you have acquired US (foreign) Citizenship on or before 31 May 2010 and if you have "Cancelled" stamp on your Indian passport, then there is a no requirement of acquiring "Surrender Certificate". If you do not have a cancelled stamp on Indian passport then you need to get the "Surrender Certificate". However fees for that is US$ 20/- only. For all other naturalized citizens of Indian origin surrender certificate fees will be US$ 175/-.

(iii) You can pay fees by way of Money Order (MO) / Postal Order (PO) / Cashier Cheque (CC) drawn in favour of “Consulate General of India, Chicago”. You can pay in cash, only if you are submitting your application in person at the Consulate. Please note that when you are sending application by mail then send it by USPS Express mail or FEDEX only so that you can trace your application with the postal agency. While sending application by mail, write on the top of the envelope “Attestation for PAN card application”. Please also note that the Consulate General of India, Chicago will not bear any responsibility for loss of documents during transit in mail.

Credit/debit cards/personal Cheque are not accepted. You can give a combined Cheque for attestation & mailing fees. However mode of payment should be one i.e. no partial payment by Cheque & partial payment in cash will be accepted. You can send one or more application by mail, for every additional application add one dollar mailing fee per applicant

8.   Processing Time:

Counter 0900 to 1230 hours: Delivery same day 330 pm to 0500 pm (Chicago time)

NOTE


Please also note that notarization is not required if you are submitting application in person, but in that case you have to show the original passport as well as other original documents to the Consular Official. We follow Chicago time.  The decision of the Consular Officer will be final in all cases.
